Closes #13708

Adding additional property generateFormAsModel.

When setting it to true, generate model for request form body.

When setting it to false, generate form params for request form body which is the current behavior.

Default value is false.

You may regenerate an individual generator by passing the relevant config(s) as an argument to the script, for example ./bin/generate-samples.sh bin/configs/java*. File the PR against the correct branch: master (6.1.0) (minor release - breaking changes with fallbacks), 7.0.x (breaking changes without fallbacks)
