As mentioned in #5546, the Swift URLSession implementation to do file uploads is not correct, it doesn't follow the multipart spec. The differences with the spec are:
- One newline too much between headers
- One newline too much after the body
This PR fixes that.
A lot of other things have also changed because I ran ./bin/swift5-petstore.sh. I guess the previous change did not do that.

Copy the technical committee to review the pull request if your PR is targeting a particular programming language.
