Feature: Generate table of contents during build

Summary

We should generate the table of contents at build time, rather than relying on the client device to generate it. This will allow users to benefit from the TOC without requiring JavaScript, and will lessen the payload of the a11y project overall.

Luckily, there's prior art for this by way of eleventy-plugin-toc
